Waiver / liability release

  1. Voluntary Participation: I, {name}, voluntarily choose to participate in activities at Ambassadors of the Art BJJ LLC (“AOA BJJ Academy”) at the facilities of 4601 Hondo Pass, Suite i, El Paso, TX 79924. I understand that Brazilian Jiu-Jitsu involves strenuous physical activity and close contact, which may include joint locks, throws, chokes, ground fighting and techniques, and similar movements. I acknowledge that participation involves inherent risks of injury.
  2. Assumption of Risk: I understand and accept all risks associated, including the possibility of injury or death. I participate entirely at my own risk and responsibility.
  3. Release of Liability: In consideration for instruction, use of the facility, and participation in the Academy activities, I release and hold harmless Ambassadors of the Art BJJ LLC, its owners, staff, instructors, members, agents, and affiliates from any and all claims, demands, or liability for injury, death, or property damage arising from my participation, including those caused in whole or in part by negligence.
  4. Media Release: I understand that classes or events may be photographed or recorded. I grant AOA BJJ Academy permission to use my image for promotional or informational purposes without expectation of compensation.
  5. Health & Fitness Acknowledgment: I confirm that I am physically capable of participating in the activities and will follow all rules and instructions. I acknowledge that Brazilian Jiu-Jitsu involves close physical contact and may involve risk of transmission of illness. I hereby represent that, to the best of my knowledge, I am in good physical health and condition, sufficient to undertake the activities and practices required.
  6. Indemnification: I agree to indemnify and hold harmless Ambassadors of the Art BJJ LLC, its owners, staff, instructors, members, agents, and affiliates from any claims or expenses (including attorney’s fees) arising from my participation, use of the facility, or use of techniques learned at the Academy.
  7. Knowing & Voluntary Execution:  I have carefully read and fully understand this waiver. I acknowledge that I am giving up legal rights, including the right to sue. I acknowledge that this waiver is a binding contract and recognize that I am forever waiving, among other things, my rights to sue Ambassadors of the Art BJJ LLC, or any of its subdivisions thereof, its owners, directors, officers, instructors, lessees of premises, managers, employees, agents, promoters, consultants, and members for injuries, damages or losses that I may incur while participating  This agreement is binding on me and my heirs, representatives, and successors. I sign this waiver voluntarily and of my own free will.
